A successful dementia program cycle has to be measurable, easy to understand/execute, and must meet the actual needs of the population it serves.
1. Start with a comprehensive needs assessment including residents, families, and caregivers.
2. Determine what measurable outcome best matches your population's needs.
3. Complete baseline staging assessments followed by appropriate therapy referrals
4. Transition to caregiver team who has been educated on cognitive assistance strategies developed by the therapy team through the rehab episode
5. Review changes in the measurable outcomes established in Step 2 routinely to determine when to repeat the cycle
